REACH Counseling & Wellness, LLC provides therapeutic services which foster an environment that promotes Resilience, Empowerment, Awareness, and Confidence utilizing a Holistic approach to wellness.


Treatment philosophy

Client-centered: You and the therapist together will determine goals for therapy and decides interventions that will best fit your needs. Building a positive, trusting team between the client/family and the clinician is one of the most important aspects of effective treatment.

Strength Based: Therapy focuses on the strengths of each client that can be used as tools to continue healing themselves. This approach builds confidence and resilience in order to overcome obstacles they may face.

Holistic: Treatment will incorporate education on healthy living, including proper diet and exercise, to establish and maintain optimal mental health.

Evidence-based/Outcome-oriented: Treatment approaches have been proven effective through research. Your therapist will continuously evaluate whether the interventions are working and, if not, change strategies to get better results.

Efficient: Your therapist strives to achieve goals while using the least amount of resources as possible. This includes minimizing cost to the funder and family for services, as well as designing interventions that are both cost-efficient and energy-efficient (least effort) for the client/family.



What to Expect:

Client will meet with their therapist initially to complete an intake session where basic contact information and a detailed personal history will be discussed about the client in order to gain a better understand of how to best treat the client.

Sessions are scheduled on a weekly basis, but may be increased, depending on the seriousness of the issues.

Within the first month, you and others you may choose to include in treatment, will work with your therapist to develop a treatment plan, which will list your goals for treatment and ways to reach those goals.

Every 3 months, your treatment plan will be reviewed to assess progress made towards your goals and re-evaluate your goals if needed.

Some adventure-based techniques (including interactive activities inside or outside) may be included in the treatment process in order to assist you with gaining a better understanding of personal struggles and ways of overcoming them.

By the end of treatment, you should be feeling better and you should have met your goals that you identified at the beginning of treatment.

At the final session, your therapist will make sure that you are prepared for independence and have an overall sense of health and stability. You will be asked to evaluate the success of your treatment and provide input about your satisfaction with services provided.

Several months after treatment ends, you may be contacted as a follow up to make sure you are still doing well.


Your Rights:

Respect: You and your family will be treated with respect.

Anti-Discrimination: Your religious and cultural beliefs will be respected. You will not be treated differently based on your age, disability, race, sex, or ethnic group.

Freedom of Choice: You have the right to change providers, refuse referrals for other services, or stop services at any time, without penalty.

Confidentiality/Privacy: You have the right to decide when and with whom to share your private information. Information about you will not be shared with other people without your permission (or your parent's, if you are under 18). There are exceptions to this rule, including if we have to report suspected abuse, neglect, or exploitation, if we believe someone's life is in danger, or if a judge orders it. Information shared in an individual session with a child may have to be shared with a parent, but the clinician will only share information if it is necessary for treatment.

Access to Records: You have the right to read and have copies of your chart information in a timely manner. We might require that we be present to explain what is written, however, and there may be a cost for some records. You have the   right to access medical information that may be used to make decisions about your care. Usually, this includes medical and billing records, but does not include psychotherapy notes. To access medical information that may be used to make decisions about you, you must submit your request in writing to the address listed. If you request a copy of the information, we may charge a fee for the costs of copying, mailing or other supplies associated with your request.

Grievances: If you make a complaint or file a grievance, your services will not be terminated or affected in any way.

Freedom from Harm: We are required by law to report any suspected abuse, neglect, or exploitation. If you need to report abuse, neglect, or exploitation, call the Abuse Hotline: 1-800-96-ABUSE (800-962-2873).

Social Media & HIPAA Regulations: Input and participation is encouraged, however, any complaints or comments posted on social media regarding patient care for which you request a response must be directed to REACH Counseling & Wellness, LLC, in compliance with HIPAA and other laws and regulations.  REACH Counseling & Wellness, LLC is unable to respond to any specific patient care comments on social media and reserves the right to remove any comments which   may represent a breach of HIPAA or applicable laws/regulations.



Your Responsibilities:

Attendance: You will keep appointments made with the therapist . If you cannot keep an appointment, call your therapist ahead of time to reschedule.

Participation: You will participate in treatment and will follow through with the strategies agreed upon during treatment sessions. Participants understand that during the therapy process they may experience successes, failures, risks and uncertainty given that the nature of experience is not entirely predictable.

Notification: You will inform the therapist of any changes to your address or phone number should your therapist need to contact you.

Payment: You are responsible for any services, co-payments or deductibles not paid by your insurance.



Agency Rights & Responsibilities:

The therapist will behave in a professional manner, including being trustworthy, considerate, polite, and discrete.

Any adventure based activities will be planned and conducted with the participants best interest in mind. Staff will monitor the physical and emotional impact of adventure programming on participants and modify the programming to best address the participants' needs, interests and abilities.

Consistent, high quality, professional treatment will be provided to you and your family. Should a session need to be cancelled and/or rescheduled for any reason, the therapist will notify you in advance.

Accurate records of the treatment being provided will be kept in a secure electronic health record system.

The therapist has the right to terminate services if you fail to follow through on your responsibilities (listed above) or if they believe that maximum benefit has been reached. If your treatment is discontinued and you still need help, you will be given a referral to another agency that can help.
